New resources from The National Institute for Health and Care Excellence (NICE)

NICE has published two new resources to support implementation of the guideline on older people with social care needs and multiple long-term conditions. Both resources were developed by the NICE Collaborating Centre for Social Care (NCCSC), a partnership led by SCIE.

 

Tailored resource: The named care coordinator role from the perspective of older people

This resource presents the views and expectations that older people have of the named care coordinator role, based on focus groups held by Age UK Sutton, plus case studies of good examples.

 

Roundtable report: Personal and joined up support for older people with multiple long term conditions
Practical examples of how to put the NICE guideline on older people with multiple, long-term conditions into practice.
