Empowering patients and communities

The Forward View into Action: New Care Models: update and initial support – July 2015 (section: 4.2) states the importance of  “Fully harnessing the energy of patients, local people and diverse communities requires a new model of partnership. In order to support this new relationship, the Five Year Forward View People and Communities Board, working with patients, the voluntary sector and vanguards, has set out six principles for new care models”

One of the first pieces of work carried out by the People and Communities Board, alongside the New Models of Care Vanguard Sites, was to produce a set of six principles for changing the way that health and care relate to people and communities. These ‘six principles’ set out the basis of good person centred, community focused health and care.

The six principles require that:

  1. Care and support is person-centred: personalised, coordinated, and empowering
  2. Services are created in partnership with citizens and communities
  3. Focus is on equality and narrowing inequalities
  4. Carers are identified, supported and involved
  5. Voluntary, community and social enterprise and housing sectors are involved as key partners and enablers
  6. Volunteering and social action are recognised as key enablers

If you are a VCSE organisation seeking to work in partnership with a vanguard you can use these principles to strengthen equal co-production, influence & voice between the sectors

 


Five Year Forward View People and Communities Board 

There are seven governance Boards established to implement the NHS Five Year Forward View. National Voices chairs the People and Communities Board.

The People and Communities Board exists to:

  1. Champion the Five Year Forward View vision and commitments and more broadly ensure that person-centred care and community-centred services are embedded in the Five Year Forward View programme;
  2. Harness the expertise of patients, service-users, engaged citizens, the voluntary sector, and other stakeholders, providing a support and challenge function in relation to the delivery of the commitments across the whole of the Five Year Forward View, including those that relate to prevention and new models of care.

The six principles for new care models
