New Care Models: Vanguard Sites in the East Midlands and East of England
The East Midlands and East of England Vanguard Sites are part of what is known as the New Care Models Programme, a new initiative by NHS England to support the implementation of its The Five Year Forward View (5YFV)
What’s happening in my region?
There are Nine Vanguard sights in the East Midlands and Two in the East of England. Over the coming months, One East Midlands and Wellbeing East, along with the other regional networks that make up Regional Voices, will be linking up with vanguard sites across the country to establish who is involved, what they are doing and what the rest of the sector can learn from these sites.
What will the Vanguards be doing?
Each vanguard site will take a lead on the development new care models which will act as the blue prints for the NHS moving forward.
Integrated Primary and Acute Care Systems – will join up GP, community, mental health and hospital services. Under this new care model, primary and acute care systems (PACS) would provide list-based GP and hospital services, together with mental health and community care, in single NHS organisations for the first time.
Multi-specialty Community Providers – will aim to move services out of hospitals and into the community. Under this new care model, GP group practices would expand, bringing in nurses and community health services, hospital specialists and others to provide integrated out-of-hospital care. single NHS organisations for the first time.
Enhanced health in care homes – will aim to improve services for older people, joining up health, care and rehabilitation. Under this new care model, the NHS will work in partnership with care home providers and local authority social services departments to develop new shared models of care and support, including medical reviews, medication reviews and rehabilitation services.
Urgent and emergency care - These sites will test new approaches to delivering urgent care and aim to improve the coordination of services and reduce pressure on accident and emergency departments.
Who are the Vanguards in the East Midlands and East of England?
Integrated primary and acute care systems (PACs)
Mansfield and Ashfield, Newark and Sherwood CCGs
This programme is called Better Together shaping health and care in Mid Notts.
Health and Social Care Services across Mid-Nottinghamshire have joined together to develop the Better Together programme. The better Together programme board includes a Voluntary Sector Special Purpose Vehicle of three District Council CVS’s: Ashfield Voluntary Action, Mansfield CVS and Newark & Sherwood CVS
This model in particular is intended to redefine the relationship between primary and secondary care
Multispecialty community providers (MCPs)
Derbyshire Community Health Services NHS Foundation Trust
The Vanguard will develop a prevention team made up of health and care professionals including GPs, advanced nurse practitioners, mental health nurses, extended care support and therapy support.
By working in close partnership and collaboration with several local NHS providers, key elected authorities, local social service providers and the voluntary and community sector, Working in multidisciplinary teams, Lakeside Healthcare plans to offer a number of new services including, ‘extended primary care services’ to most patients; providing convenient care every day of the week.
GP practices in Rushcliffe have come together to establish a new and unique primary care partnership and organisation, which will lead on and own the transformation of general practice. The new model of integrated care is focussed on early intervention, living well at home and avoiding unnecessary use of the hospital. Care will be delivered closer to patients’ homes resulting in an enhanced experience and improved clinical outcomes, and better use of available resources.
Models of enhanced care in care homes
Nottingham City CCG’s application is to work collaboratively to provide models of enhanced care in care homes and was submitted in partnership with CityCare Partnership, Nottingham University Hospitals NHS Trust, Nottinghamshire Healthcare NHS Trust, Nottingham City Council, AgeUK Nottingham and Nottinghamshire and local primary care providers.
The proposed new model will provide a structured and pro-active approach to care, complemented by a number of local innovations and will build on work already initiated with acute trusts, care homes and community services to develop a ‘pull’ approach to acute discharges, ensuring that social services are involved at the earliest opportunity
Urgent and emergency care
Great Nottingham System Resilience Group
The vanguard will support ambitious improvements in urgent and emergency care for the citizens of South Nottinghamshire. Partners will look at what more they can do, using innovative workforce solutions to ensure that people receive care in a timely way and closer to home – in many cases avoiding the need for assessment or admission to hospital.
Leicester, Leicestershire & Rutland System Resilience Group
The Vanguard will create a new alliance-based urgent and emergency care system where all providers work as one network. This will bring together ambulance, NHS111, OOH and Single Point of Access services to ensure that patients get the right care, first time.
Acute care collaboration vanguards sites
East Midlands Radiology Consortium (EMRAD) (Radiology single-specialty network)
East Midlands Radiology Consortium (EMRAD) is a consortium of seven NHS trusts within the East Midlands working together, hosted by Nottingham University Hospitals NHS Trust. Together they aim to create a clinical network, providing timely and expert radiology care for patients across the East Midlands regardless of their location
Working Together Partnership (South Yorkshire, Mid Yorkshire, North Derbyshire)
The Provider Working Together Partnership is an existing partnership established in March 2013 between seven acute trusts in South Yorkshire, Mid Yorkshire and North Derbyshire. The programme aims to develop a clinical strategy involving different models highlighted in the Dalton Review.
In the East of England there is one Vanguard site operating under the enhanced care in care homes model
East and North Hertfordshire CCG
The Vanguard is made up of Hertfordshire County Council, East and North Hertfordshire Clinical Commissioning Group and Hertfordshire Care Providers Association and will focus on enhancing the skills and confidence of care home staff through a package of education and training.
The programme will create dedicated multi-disciplinary teams for Care Homes which will include GPs, community psychiatric nurses, district nurses and geriatricians.

Cambridgeshire and Peterborough Clinical Commissioning Group
As a UEC Vanguard the CCG aims to accelerate improvements and develop a best practice model for urgent care services which helps address variations in access to services and health inequalities in the region.
