New webpage for the Wellbeing East Network

The Wellbeing East Network has a new page called Wellbeing East: Health & Social Care Reform and the NHS 5yr Forward View

The new webpage features details on a range of national programmes which are changing the way services are delivered, to align every-day practice with the vision set out in the five year forward view (5YFV) including information on what's happening in the East of England around:

• New care models - ‘vanguard’ sites
• Success regimes
• Integration Pioneers
• Integrated personal commissioning (IPC)
• The prime minister’s GP access fund (formerly known as the prime minister’s challenge fund)
• Diabetes Demonstrator sites
• Devolution
